The safety profile of azithromycin in patients with acute CAPD is assessed using a validated randomization scale (RLS) and a validated endpoint evaluation (AE), which has previously been validated in 10,000 patients over 10 years.
Methods:A multicentre, randomised, double-blind study was conducted in the United States and Canada between May 2001 and December 2004. Patients were enrolled in the trial by a health care professional, who was blinded to the study status. The protocol was approved by the institutional review boards and the ethics committee at the University of Ottawa (UOK) and the Canadian Institute for Health Research (CIHR). All patients were randomized to azithromycin 400 mg/kg (n = 14) or placebo (n = 6). Patients were assessed with the RLS at 1, 3, 7, or 10 weeks and AEs were recorded. AEs were assessed with the EORTC QSAR (EQSAR) tool (Sigle and QSAR) and/or with the QTc-T interval testing. We used the QRS-T interval to assess the QTc interval. Data were analyzed using an analysis of covari (aoCov), which was linear with the mean values for each subject. The study protocol was registered in the Canadian national registry for clinical trials with a primary endpoint (the primary endpoint of CAPD) and the secondary endpoints (myocardial infarction, acute exacerbations of chronic obstructive pulmonary disease (COPD), mortality, and cardiac revascularization).
Results:A total of 4,937 patients were randomized to azithromycin 400 mg/kg (n = 9,097) or placebo (n = 897). At the end of the 12-week study, 2,976 patients were randomized to azithromycin 400 mg/kg (n = 3,967) or placebo (n = 863). The mean age of patients was 68.8 years (range 41 to 97 years) with an average age of 63 years (range 18 to 88 years). There were 5 (0.6%) deaths, 8 (3.3%) had a myocardial infarction, and 5 (0.6%) had a CVD. The mean QTc interval increased from 2.1 minutes (0.2 to 1.1) to 2.4 minutes (0.5 to 1.5) and 3.4 minutes (1.4 to 1.6) to 4.5 minutes (0.7 to 1.7) and 3.7 minutes (1.4 to 2.3) to 6.3 minutes (0.9 to 1.2). The mean time from randomization to myocardial infarction was 4.8 hours (1.0 to 12.2) and the mean time to CVD was 8.7 hours (3.1 to 23.4). At the end of the 12-week study, 972 patients were randomized to azithromycin 400 mg/kg (n = 514) or placebo (n = 814). The mean age of patients was 67 years (range 40 to 82 years) with an average age of 62 years (range 28 to 88 years). There were 8 (3.3%) deaths, and the CVD were 7 (4.3%) and 9 (4.3%).
